- You've made the diagnosis, let PEAK handle the rest!
- Elimination of hospital burden by referring a case directly to PEAK for consultation and surgery.

- Improved patient/client care by a direct referral when your OR is too busy to squeeze in another surgical procedure.
- Additional services are easier to schedule with a direct referral such as platelet rich plasma (PRP).
In-House Mobile Surgery Referral
DIAGNOSIS:
- A diagnosis is made and surgery discussed with the client.
- PEAK also offers surgery teleconsults to review surgery and recovery in detail with owners.
ESTIMATE:
- Contact PEAK to request a surgical estimate or you can refer to the current price list (available upon request).
- Add your hospital fees and present to the owner.
SCHEDULING:
- After estimate approval, email PEAK 3 available surgery dates along with a fully completed "Surgery Referral" form.
- Once PEAK has received the "Surgery Referral" form, your selected surgery date is secured!
DIAGNOSTICS:
- A CBC and Chemistry is required for all procedures unless otherwise discussed.
- Three view thoracic radiographs are recommended for all cases over 6 years of age and required for any respiratory or neoplasia cases.
- ALL orthopedic radiographs MUST include a calibration marker.
- All diagnostics must be current (within 4 weeks of surgery) and emailed to PEAK no later than 3 days prior to surgery.
DAY OF SURGERY:
- The patient should be fasted and dropped off by 7-7:30am.
- The appropriate "Surgical Release/Consent" form should be fully completed and signed.
- Dr. Savicky will provide surgical supplies and perform surgery.
- After the surgery is completed, Dr. Savicky will call your client directly to discuss how surgery went and review discharges in detail.
- PEAK will provide you with a surgery report and discharges.
FOLLOW-UP:
- Suture removal and recheck radiographs (if needed) will be performed by the primary care hospital.
- Dr. Savicky will be happy to evaluate radiographs and answer any questions as needed.
Direct Consult/Surgery Referral
DIAGNOSIS:
- A diagnosis is made and the primary care veterinarian recommends a direct surgery referral for consultation and surgery.
- PEAK accepts cases for direct referral at 2 convenient locations:
- Arcadia - 36th Street & Indian School Rd.
- North Scottsdale - 101 N & E Raintree Dr.
ESTIMATE:
- Contact PEAK to request an estimate.
SCHEDULING:
- Email PEAK a fully completed "Surgery Referral" form.
- Provide your client with PEAK's contact information and have them call/email to schedule the consult.

DIAGNOSTICS:
- Email any current (within 4 weeks of surgery) diagnostics & pertinent history:
- Labs, cytology, radiographs, pertinent history (avoid sending entire patient records)
REFERRAL SUMMARY:
- PEAK will perform the consultation and if elected, surgery.
- The consultation summary and/or surgery report with discharge will be emailed to your hospital.
